Aspire Together Upstate, LLC, provides conflict free Service Coordination through the Nursing Home Transition and Diversion Waiver and the Traumatic Brain Injury Waiver under the NYS Department of Health.

Service Coordination is an individually designed service which provides primary assistance to the individual in gaining access to needed waiver and Medicaid state plan services as well as other local, state and federally funded educational, vocational, social and medical services. The Service Coordinator provides advocacy and choices of services in order for the individual to live their best life.
